We have an open list and welcome requests for registration from patients living in or moving to the practice area.
You can register by completing a form available from reception. You will need to complete this by hand.
You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.
Permanent patients may express a preference to receive services from a particular doctor or nurse, either generally or in relation to any particular condition.
Whilst we shall endeavour to comply with any such preference we reserve the right to refuse such services if the Doctor/Nurse does not routinely perform the service in question within the practice.
Proof of Identity for New Patients
Patients registering with the Practice will be asked for proof of identity. A list of acceptable documents is given below.
- birth certificate
- marriage certificate
- medical card
- driving licence
- local authority rent card
- paid utility bills
- bank/building society cards/statements
- National Insurance number card
- current employment payslip
- letter from Benefits Agency/benefit book/signing on card
- identity papers from the Home Office
